Regarding endodontic microsurgery, beyond the necessity of a complete surgical set-up and adequate operator training, the treatment indication seems to be divided between last chance attempts - the last station before extraction and surgical treatment indications for lesions of endodontic origin even where orthograde endodontic retreatment offers good success rates.

The present presentation tries to establish some guidelines and principles of treatment, removing some stereotypes according to which the surgical approach is a low-cost approach to an endodontic retreatment or a dental implant, or that it is a palliative approach that gives only a short-term delay of the extraction or that it is conditioned by the size and nature - for example cystic - of the lesions.

Due to the irreversible and perceived invasive nature of the surgical treatment, as well as the increasingly aggressively promoted "competition" of implant therapy, a judicious selection of the case and a very good communication with the patient is necessary.

Finally, the success rate of endodontic microsurgery will be debated, cases from our own practice proving that when case selection is correct, a good prognosis is possible even in those cases generally viewed with skepticism such as addressing lesions on lower molars or of teeth with large apical processes that affect both bone-plates "through and through lesion".

Direct restorations with composite are challenging in daily practice due to their particularities. A predictable and consistent protocol, tested in daily practice, is the key to successful restorations on a long term. In the theoretical part we will cover all intra-operatory steps, isolation, adhesion, modelling, while in the practical part we will work through each step with the help of the new advanced universal composite with unishade simplicity, G-aenial A’CHORD, from GC.

The traumatic fracture of the anterior teeth represents a quite common pathology in our daily practice. In some situations, it is contraindicated to use indirect restorations (in the cases of very young patients, class III restorations, ultra-conservative restorations, emergency and low cost treatments). However, it is necessary to be able to restore these teeth in a direct, aesthetic, functional and predictable way. We will have to focus on the restoration of the color as well as on the restoration of the morphology.        
   
Due to some factors related to the characteristics of the composite resins, depending on the patient and the dentist respectively, the quality of the direct anterior restorations may decrease with time. Not understanding the causes that lead to these changes over time, makes most of the practitioners avoid this type of restorations.           
This presentation brings up the steps and the techniques that the dentist has to follow in order to maintain the aesthetics of the direct anterior restorations as long as possible.

Since the inception of dental implants in mid ’70 in Sweden there is little advancement in dental implants. Now nearly 50 years after we are finding over 45% cases develop Peri-implantitis in 7-9 years and 15% of cases have to be extracted! Such findings are Not published in Implant journals. Many believe implants are for life time and trouble free. Studies clearly showed that implants have lots of problems and not for permanent solution. 
In the same period, advancement of endodontics both in non-surgical and surgical approaches has been phenomenal. Stainless K-files changed to NiTi rotary files and now 3D files, incorporation of Microscope, X-ray to CBCT, ZOE cement to Bioceramic sealer and putty and Digital stent tech etc. All these advancements are designed to save teeth in one’s mouth. Fortunately, this mission has been accomplished because if teeth are firm, no or little mobility and no perio pockets all teeth can be saved. It is totally different generation of Endodontic Therapy. In this presentation all the advancement will be presented and compare with implants.

Bioceramic materials are one of the last novelties in Endodontics and they seem extremely interesting products thanks to their chemical and physical properties and to their biocompatibility. They seem also easy to use and adaptable to different clinical scenarios, from the cold obturation with a single cone to the modified warm guttapercha compaction techniques, with a special mention to the bioceramic putty dedicated to the endodontic microsurgery. The difficulty in the clinical practice is to learn how to manage these materials, how to apply them correctly and how to make the best out of their chemical and physical properties. A dental student or a dentist can reasonably have the doubt to use these new materials or to stick with techniques that have a longer history of success. The Literature has not produced yet clear guidelines and long term results on the clinical use of bioceramic materials, despite their increasing use in current clinical practice, hence the need to put face to face the research and the clinics. The aim of this lecture is to compare the Literature and the Clinical experience on bioceramics, putting together the best studies on this topic and the clinical case reports that will suggest at the end how to achieve predictable results with “bioceramics”.

The aim of this workshop is to introduce dental clinicians in using the Palodent Plus sectional matrix and centripetal techniques for direct restoration of class II cavities. Participants in this workshop will learn how to properly restore the contact points for posterior teeth. Each participant will exercise how to obtain a correct contact between molars. Participants will apply the centripetal technique for the restoration of the proximal wall and successive cusps technique to recreate the occlusal morphology.

Obturation is a very important step of the root canal system in order to achieve a positive outcome of the therapy; as a matter of fact a high percentage of the endodontic failures are associated with incomplete root canal obturation. Warm gutta percha techniques are certainly the best techniques to achieve the obturation objectives and can be considered the “golden standard” techniques to get a high quality obturation. Lately, the introduction of the Bioceramic sealer changed the obturation scenario. Bioceramic sealers, as the CeraSeal, represent a real game changer; thanks to their properties it is possible to rely on a sealer centered obturation, moving from warm to cold obturation techniques, getting an high quality obturation.. In both cases, to carry out a proper obturation is really important that the clinician knows the technique and the characteristics and/or properties of the materials he is using, as sealer and obturation devices.

The aim of this workshop is first to analyse characteristics, benefits and limitation of the different techniques in order to create a rationale for the selection of a specific obturation technique according with the clinical situation. The second aim is to share with the attendees the clinical experience of the speaker on Warm Gutta Percha techniques, analyzing details and sharing tips on the proper use of the EQ-V system, never forgetting that nowadays the Single Cone - thanks to bioceramic sealer - has become a very interesting option when it comes to carry out high quality root canal obturation.

The presentation aims to encourage the participants to use the magnification offered by the dental operating microscope and its use for direct restorations.

To get the benefits of the Bioceramic sealers, the clinician does know their features, the limits of use and when there is a strong indication to their use.

The aim of the lecture is to analyze all the features of the sealer to understand how to use it properly in order to get in the clinical practice all the benefits of this sealer. During the lecture will be discussed even some interesting points regarding the bioceramic sealer, as the retractability, the possibility to be used with warm techniques and the impact on the outcome of therapy. At the end of the lecture the participants will be able to recognize and comprehend the importance of a high quality obturation to get a positive outcome of the therapy, to understand the importance of the knowledge of the bioceramic sealer for its proper use during the obturation and to recognize that the cold hydraulic condensation is a valid technique even in presence of big apices or small perforations

Endodontics is the discipline that deals more than the others with the care of teeth that cannot be kept alive or have undergone an unsuccessful treatment.  Even if Endodontics has made enormous progress today, with equipment, instruments, microscopes, lasers, it is not always possible to solve every problem. Sometimes extraction is required. But the evolution of thought, of study, of in-depth analysis, allows us to have a much more cautious, more conservative attitude. Today we know and have codified that we can use extractable root parts to achieve better bone morphology, implant support, better gingival levels, and more.

We also know that when a tooth has to be extracted, we don't necessarily have to resort to something artificial, but, after having evaluated all the possibilities, we can take into consideration, the replacement of the lost tooth with another tooth, which, for various reasons, it does not have such an important role or function as the lost one, and gives us the opportunity to replace it in a natural, biological way. This technique has numerous advantages, as well as being the most natural solution, it is particularly suitable before complete body maturation, when implants are strongly contraindicated. It is a much cheaper solution with very interesting social implications. Let's say that it is an often underestimated alternative.

I would add that the specialist who knows best how to make these decisions is the endodontist, who has the last word on the possibility of saving a tooth and has the knowledge for the morphological evaluation and the adequacy necessary for replacement. We will try to give all the parameters recognized by the literature to be able to carry out these techniques in absolute safety and when they reasonably present a plausible solution.
